This is done by either keeping records of contact information or keeping in touch with shelters and moving to the streets or homes to find missing patients.
One of the most basic barriers is that some patients don't have a phone/contact to keep easy and constant follow-up with. Another one of the slightly more complex issues is the patients who use crack-cocaine and are not particularly interested in care at this time.
Efficiency should increase once BHCH has more efficiently gathered data from the current EMR system.
