The Mount Sinai clinics use telephone reminder calls to help patients attend appointments. Calls were piloted at varying times, and the ultimate time and frequency of the calls was determined via CAB recommendations.
Registrars at the reception desk are asked to verify active phone numbers and addresses at each visit, to ensure that outreach efforts are as efficient and effective as possible.
Metro Cards are often provided to help overcome transportation barriers.
The "Care Coordination Program" has made return-to-care efforts for patients who meet specific program inclusion criteria. These efforts will be renewed, and greater efforts to document whether patients have moved, transferred care, or died will be made, beginning with clients identified as “out of care” via the SPNS initiative pre-work data tools.
