We continue to utilize the role of the Promotora or Retention Specialist todo phone calls to remind the patients of their appointment or make or givean appointment to those that have not been seen for care.
We have added more staff to assist in reviewing the electronic healthrecords of the patients that are out of care.
Based on the retention formula, we are provided a 12-month rolling reportfrom our IT department of patients that have not had a visit for 6 months ina 12-month period. This report enables us to identify the patients for phonecalls so as to bring them back into care.
Barriers/Challenges: Biggest challenge remains keeping up to date on correct contact information for patients in system.
