• We’ve created a tool to identify patients out of care. There were 37 patients who’s last date of service was =>120 days. We were able to determine the status of 26 of the 37 patients (70%). 24 are considered exclusions. 1 patient’s phone was disconnected and another patient scheduled an appointment.
• We are currently determine the status of 38 patients who’s last date of service =>90 days. If they chose to stay with our organization, we will assist them with getting an appointment. If not, we will assist them in transferring their care to a different agency.
• The patients don’t like the long wait time to see a provider, particularly if they don’t feel sick. We have suggested that they at least come in for the routine lab work and if a provider visit is necessary, they will be contacted.
• Another issue is timely documentation of the patient status in the patient medical record and deliberate communication to all HIV team members.
